Computer-Aided Triage

Also called: CADt

Software that reprioritizes a worklist by flagging studies with suspected time-critical findings, without marking images or offering a diagnosis.

CADt devices change the order in which studies are read. They do not mark the image, do not remove studies from the queue, and do not produce a diagnosis. The clinical claim is time to notification, not accuracy.

The category was created by the De Novo authorization of a stroke triage device in 2018 and has since become one of the largest groups on the FDA list.

Where This Gets Misread

Triage devices are evaluated on sensitivity and notification time, not diagnostic accuracy, so a high false positive rate is compatible with clearance. The benefit also depends entirely on there being a queue: in a department that reads studies promptly anyway, reordering the queue achieves nothing.
